All Information Requested Must Be Provided To

If we establish an account for you your terms will be "net 30 days"
unless otherwise stated on each invoice. Invoices not paid within
terms will be subject to a 1.5% finance charge which is assesed on
the 25th of each month. Our minimum invoice is $5.00. We also require
checks to have the address and phone number imprinted on them.

All information submitted above is true and correct, and, if
an account is established, terms outlined above are accepted.

We will email you back with a
copy of this document and ask you to confirm it's validity by
emailing it back once again to us. In lieu of an actual
signature this is the only way to confirm the validity of the
application and to insure against potential fraudulent submissions.
